If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification. You can find guidance on self-certification and an online form you can complete and print to send / give to your employer – by clicking here

Please note, completing this form does NOT guarantee the GP will be able to issue you a note, they may still need to have a consultation with you – in which case you will be contacted and asked to book an appointment.
NB: if you have an existing sick note and are asking for an extension, we cannot take these requests BEFORE your current note runs out. They can be back dated but not dated in advance.
